YOU MUST MEET ALL OF THE FOLLOWING REQUIREMENTS TO QUALIFY
- My family meets the income requirements indicated in the box to the right.
- I am the only person enrolling my children in any gift-giving program.
- I am requesting gifts for my children ages 18 years and under.
GIFTS ARE FOR CHILDREN IN MY HOUSEHOLD ONLY.
We request a $1.00 DONATION for each child. (If you cannot donate when you sign-up, you may donate when you pick up the gifts. We will gladly work with you if cannot afford to donate.)
Families may return their forms to:
- Mat-Su Special Santa Workshop after Monday, November 28th from 1:00 to 5:00.

Family Gift Pick-Up Location: 720 N. Shoreline Drive off of Palmer-Wasilla Hwy. Turn at the light on Shoreline between Health Quest Therapy and Town & Country Furniture Store. (Follow the Signs)

***Friday, December 23 - Late Sign Up or Drop-Ins from 10:00 to 4:00 we will assist families who did not sign-up.
